New growth on cuttings turning black

The good news is the roots look great.  They are really growing nicely.  Bad news is the new tender growth on 2 of them is turning black.  I've had this happen once before and would like some advice.  I have perl/verm mix and it is not very wet.  Could there be too much humidity in my container?

Any advice is appreciated.

I would assume that humidity is too high and it is mold.

I have the same problem. I had the cuttings under plastic with a little moisture on the plastic but not wet and they were starting very nicely. I took the plastic off and that is when the small leaves turned black. Any thoughts?

I would assume the leaves dried out as the root system was not strong enough to keep the leaves alive. Don't pull the cutting out, but do you see roots growing?

I've had it happen too.  The tip and/or leaves turn black and die.  I've had one of two things happen.  Either new leaves will grow from the tip or new branches will form at the base of the shoot with the dead leaves. 
I'm looking at one now.  I've got it growing in a narrow plastic bag.  The leaves touched the side of the bag that had too much humidity and overnight the leaf turned to mush.  I don't think this one died from the tip, but time will tell.
