Hi I'm trying to fill out this uc davis form and I haven't a clue as to what they mean by the words "GENUS" and "FORM, any help would be greatly appreciated. thanks!!
Genus is Ficus, the form you are after is dormant cuttings. They only send out dormant cuttings

Wasn't sure about the ficus but I was thinking that the form was about green or dormant cuttings, thank you for your response Brent.
